Asthma is a chronic respiratory condition that can cause recurring episodes of wheezing, coughing, chest tightness and breathing difficulties. Although effective treatments are available, gaps in diagnosis, medicine access, inhaler technique and long-term follow-up can increase the risk of uncontrolled symptoms and severe attacks. Ensuring that patients understand their treatment and receive appropriate care is therefore an important part of asthma management.

The World Health Organization (WHO) has highlighted patient safety as a central consideration in its forthcoming asthma guidelines for primary healthcare. The updated guidance is expected to be published in early 2027 and aims to strengthen evidence-based asthma care while reducing avoidable harm. The announcement was made on 17 September 2026, in connection with World Patient Safety Day 2026, whose theme is “Safe care for noncommunicable diseases.”

According to WHO, asthma affected an estimated 363 million people globally in 2023 and was associated with approximately 442,000 deaths. Most asthma-related deaths occur in low- and lower-middle-income countries, where underdiagnosis, undertreatment and limited access to essential medicines remain challenges.

The WHO highlighted the importance of addressing airway inflammation rather than relying only on medicines that provide short-term symptom relief. Reliever or bronchodilator inhalers can ease breathing by opening narrowed airways, but frequent use may indicate poor asthma control and an increased risk of severe exacerbations. Inhaled corticosteroids help reduce airway inflammation and lower the risk of severe asthma attacks and asthma-related mortality. However, these medicines remain underused in several settings.

Access to inhaled medicines is another important patient-safety concern. WHO reported that, in 2023, bronchodilator inhalers were available in fewer than two-thirds of public primary healthcare facilities in low- and lower-middle-income countries, while steroid inhalers were available in fewer than half. Limited availability and affordability can influence treatment decisions and restrict patients’ access to recommended therapies.

The Global Asthma Network reported that approximately 45% to 60% of people with severe asthma symptoms, depending on age group, were not using inhaled corticosteroids, with larger treatment gaps in lower-income settings. Professor Refiloe Masekela, Co-Chair of the Global Asthma Network, emphasised that effective medicines must be accessible and used correctly to reduce symptoms and asthma attacks.

WHO also stressed that safe asthma care involves more than prescribing medicines. Patients need clear guidance regarding the purpose of each inhaler, correct inhaler technique, recognition of worsening symptoms and when to seek medical assistance. Healthcare professionals have an important role in demonstrating inhaler use, reviewing asthma control and supporting treatment decisions during follow-up visits.

The organisation’s World Patient Safety Day recommendations emphasise shared decision-making, correct use of medicines and medical devices, continuity of care and follow-up after severe asthma attacks. Through its forthcoming guidance, WHO aims to support safer, evidence-based asthma management and improve access to appropriate care for people living with asthma.
