Uttar Pradesh is strengthening specialist healthcare access in rural and remote areas by introducing a hub-and-spoke model across Ayushman Arogya Mandirs. The initiative is designed to connect patients visiting village- and panchayat-level health centres with specialist doctors at larger healthcare facilities through telemedicine and digital health platforms.

Under the proposed model, medical colleges, district hospitals and community health centres can function as hubs, where specialist doctors provide consultations. Ayushman Arogya Mandirs at the village or panchayat level will serve as spokes, allowing patients to access specialist services without having to travel directly to larger hospitals for an initial consultation.

At the spoke facility, a Community Health Officer (CHO) will record the patient's symptoms and upload relevant preliminary investigation reports through the e-Sanjeevani platform. A specialist based at the connected hub can then review the information and interact with the patient through a video consultation. Following the consultation, the specialist can provide a digital prescription or advise further evaluation or referral when required.

In Prayagraj, Colvin Hospital has been developed as a hub under the initiative, according to the district's National Health Mission officials. The model is intended to improve the connection between primary-level healthcare facilities and higher-level specialist services.

The programme is also incorporating point-of-care diagnostic capabilities at Ayushman Arogya Mandirs. Smart testing kits, including dry-biochemistry analysers, are being provided to support rapid testing of parameters such as haemoglobin, blood sugar and selected liver and kidney function indicators. The tests can reportedly be completed within approximately five to 10 minutes, reducing the need to send every basic investigation to a central laboratory.

Additional portable testing kits are being introduced to support the early identification of anaemia and high-risk pregnancies. These tools could help frontline healthcare teams identify patients who require closer monitoring, specialist advice or referral.

Mental healthcare is another component of the expanded digital network. Ayushman Arogya Mandirs in the initiative have been linked with Tele-MANAS services, allowing people in rural communities to seek counselling support for concerns such as stress, depression, insomnia and other mental health problems.

In Prayagraj, officials stated that 494 Ayushman Arogya Mandirs are currently operating. The district has also launched the Tele-MANAS helpline, with the numbers 14416 and 1800 made available for mental health support. According to the information provided by district officials, the service had been used by 755 patients with mental health concerns as of August 2026.

The hub-and-spoke approach reflects the broader use of telemedicine to bridge geographical gaps in healthcare delivery. Instead of requiring patients in remote areas to travel to a specialist facility for every consultation, the model allows basic clinical information and test results to be shared digitally with specialists. Patients who need physical examination, advanced investigations or hospital-based treatment can then be referred appropriately.

The combination of teleconsultation, rapid diagnostics and referral linkages could strengthen the role of Ayushman Arogya Mandirs as points of first contact for rural populations. At the same time, effective implementation will depend on reliable digital connectivity, trained frontline personnel, availability of diagnostic supplies and clear referral pathways between spokes and hubs.
