The U.S. Food and Drug Administration (FDA) has introduced temporary measures to help prevent a potential supply gap in neonatal starter parenteral nutrition (PN), which provides essential nutrients through an IV for premature and critically ill newborns who cannot receive sufficient nutrition orally or through feeding tubes.

The FDA took the step after children’s hospitals raised concerns about the availability of certain standardized neonatal starter PN products following the planned permanent closure of two outsourcing facilities manufacturing these products. The agency has issued immediately effective guidance outlining temporary regulatory and enforcement priorities for eligible state-licensed pharmacies, federal facilities and outsourcing facilities that compound specified PN products.

Starter PN can be particularly important immediately after birth for extremely premature or critically ill infants when enteral feeding cannot yet provide adequate nutrition. The FDA said it will continue monitoring supply requirements and consider further measures if necessary.To support continuity of supply, the FDA has issued immediately effective guidance outlining temporary regulatory and enforcement priorities for certain facilities that compound specified parenteral nutrition products. The policy applies to eligible state-licensed pharmacies, federal facilities and outsourcing facilities operating under Sections 503A and 503B of the Federal Food, Drug, and Cosmetic Act.

According to the FDA, the temporary approach is intended to give hospitals additional access options while longer-term supply solutions are developed. The agency said it will continue monitoring supply requirements and may take further action if the situation changes.

The issue also has relevance for India, where neonatal units regularly manage premature and very-low-birth-weight infants who may require specialised nutritional support. ICMR’s Standard Treatment Workflow for neonatal feeds and fluids states that parenteral nutrition may be considered for neonates born at less than 28 weeks of gestation or weighing less than 1,000 grams, particularly when enteral feeding is not appropriate.

India's approach also places strong emphasis on starting and advancing enteral feeding whenever clinically appropriate. ICMR guidance recommends breast milk as the preferred source of enteral nutrition and outlines specific feeding and IV-fluid strategies based on gestational age, birth weight and clinical condition.

From an Indian healthcare-system perspective, the FDA action highlights the importance of reliable neonatal nutrition supply chains, specialised compounding capacity, quality control and contingency planning. Any disruption in products used by NICUs can create additional pressure on hospitals caring for vulnerable newborns. Strengthening domestic availability of essential neonatal nutrition products, maintaining qualified manufacturing and compounding facilities, and ensuring appropriate hospital-level inventory planning could help improve resilience.

The development is therefore relevant not only to the U.S. but also to healthcare systems such as India’s, where expanding neonatal and critical-care capacity makes dependable access to specialised medicines and nutritional products increasingly important
