Advances in robotic-assisted surgery are expanding the range of complex procedures that can be performed through minimally invasive approaches. From cardiac and thoracic surgery to cancer treatment, renal transplantation and gynaecological procedures, robotic platforms are being increasingly used to provide surgeons with enhanced visualisation, instrument manoeuvrability and precision.

The growing role of surgical robotics was discussed during the latest Times Conversations on Robotics, organised by The Times of India in association with Narayana RN Tagore Hospital, Mukundapur. Specialists from multiple surgical disciplines discussed how robotic-assisted techniques are being incorporated into contemporary surgical practice.

A major focus of the discussion was the evolution of surgery from conventional open procedures to laparoscopy and, increasingly, robotic-assisted techniques. Robotic platforms can enable surgeons to operate through smaller access points while providing features such as three-dimensional visualisation, magnification, wrist-like instrument movements and tremor filtration.

In general and gastrointestinal surgery, robotic systems are being used for procedures including abdominal-wall reconstruction and complex oncological operations. Specialists highlighted the stability of the robotic camera and enhanced visualisation as useful features during lengthy and technically demanding procedures. At the same time, they emphasised that the fundamental principles of cancer surgery must remain unchanged regardless of whether an operation is performed through an open, laparoscopic or robotic approach.

Cardiac surgery is another area where robotic techniques are being explored for procedures such as coronary bypass surgery and valve-related operations. Robotic-assisted approaches can provide access through small ports and may allow certain procedures to be performed without the large chest incision associated with conventional open surgery. The discussion highlighted potential benefits including reduced surgical trauma and faster postoperative recovery in appropriately selected patients.

In urological surgery, robotic platforms are being used for procedures involving the prostate and kidney. Enhanced magnification and instrument manoeuvrability can be particularly useful when operating within confined anatomical spaces. Applications discussed included prostate cancer surgery and nephron-sparing procedures for selected kidney tumours, where preservation of healthy renal tissue is an important consideration.

Robotic-assisted techniques are also being applied in gynaecological oncology. Specialists discussed their use in procedures such as hysterectomy and lymph-node dissection, with robotic systems also supporting technologies such as fluorescence-based sentinel lymph-node identification. However, patient selection remains important, and robotic surgery is not considered appropriate for every patient or procedure.

The technology is additionally expanding in thoracic surgery, with applications including lung cancer procedures, mediastinal tumours, oesophageal conditions and selected diaphragmatic disorders. Enhanced vision and instrument manoeuvrability may be particularly useful when operating around small or difficult-to-access structures within the chest.

An important point emphasised during the discussion was that robotic surgery does not mean autonomous surgery. The robotic system is controlled by the surgeon and functions as an advanced surgical platform that can enhance movement, visualisation and precision. Clinical decision-making and operative control remain with the surgical team.

The growing adoption of robotic surgery therefore represents an evolution in minimally invasive techniques rather than a replacement for conventional surgery. Its clinical value depends on appropriate patient selection, surgeon expertise, the nature of the procedure and the availability of suitable infrastructure.
