The pricing of commonly used medical consumables has come under regulatory scrutiny in Maharashtra after the Maharashtra Food and Drug Administration (FDA) identified substantial differences between trade prices and printed maximum retail prices (MRPs) of certain hospital-use products. The findings have prompted discussion among healthcare organisations over how medical consumables should be priced while accounting for procurement, clinical infrastructure and patient affordability.

The issue gained attention after Maharashtra FDA Commissioner Tukaram Mundhe shared findings from a survey of hospital consumables in the state. The survey cited significant price differences for products including syringes, catheters and IV infusion sets. An IV infusion set with a reported trade price of ₹11.05 was listed with an MRP of ₹325, while a syringe purchased at ₹6.75 carried an MRP of ₹57.20. A catheter with a reported procurement price of ₹29.41 had an MRP of ₹310.

The findings have brought attention to the broader question of transparency and pricing of medical consumables used during hospital care. Unlike routine retail purchases, patients receiving hospital treatment may have limited ability to compare prices or select alternative products during the course of care.

In response, Nathealth, the healthcare industry's representative body, argued against applying a uniform markup to all hospital consumables. The organisation said that hospitals may incur additional costs beyond procurement, particularly when dealing with specialised medical technologies that require clinical evaluation, inventory management, sterile-chain maintenance, traceability, trained personnel, specialised infrastructure and appropriate disposal.

Nathealth called for an evidence-based and differentiated framework that distinguishes basic, commoditised consumables from clinically differentiated or high-complexity medical technologies. The organisation also stated that unusually high margins on routine consumables should remain subject to scrutiny.

The Association of Healthcare Providers of India (AHPI) similarly called for the issue to be examined within its wider regulatory and administrative context. AHPI highlighted the distinction between a product's procurement price and its MRP, noting that hospitals do not determine the MRP printed on medicines and medical products.

The discussion also involves the regulatory framework governing medical-device pricing. According to AHPI, MRPs for medical products are determined by manufacturers, while applicable price controls and annual price-increase provisions depend on the regulatory classification of the product.

The developments highlight the need to balance patient affordability, pricing transparency, legitimate hospital delivery costs and appropriate regulation of medical consumables. The debate is also relevant to the wider medical-device sector, where differences between procurement costs, distribution, hospital-level services and patient billing can influence the final cost of healthcare.
