India's precision cancer treatment landscape is set to gain a new proton therapy capability, with Fortis Memorial Research Institute (FMRI), Gurugram, planning to install the country's first upright proton therapy system. The planned installation will use the MEVION S250-FIT Proton Therapy System from Mevion Medical Systems, integrated with Marie, Leo Cancer Care's upright patient-positioning and imaging technology, and RayStation treatment-planning software from RaySearch Laboratories. The installation remains subject to applicable regulatory approvals and completion of site preparations.

Proton therapy is an advanced form of radiation treatment that uses proton beams rather than conventional X-rays. Because protons can deposit much of their energy at a defined depth in tissue, treatment planning can be used to concentrate radiation around a tumor while limiting unnecessary dose to surrounding structures. Fortis' own information on proton therapy describes the use of the proton beam's Bragg peak to deliver energy at a specific depth.

This characteristic can make proton therapy particularly relevant for selected cancers where protecting nearby healthy tissue is important. The planned FMRI system is expected to be relevant to selected adult and pediatric cancers, including tumors located close to sensitive structures such as the brain, spinal cord, heart, and lungs. However, proton therapy is not automatically appropriate for every cancer, and treatment selection depends on the tumor, its location, and individual clinical factors.

The proposed setup combines Mevion's compact proton accelerator with Leo Cancer Care's Marie upright positioning platform. Unlike conventional proton therapy systems in which a large gantry rotates around a patient lying on a treatment table, the MEVION S250-FIT uses a fixed proton beam while the patient is positioned upright and rotated in front of the beam. Marie combines an upright patient-positioning platform with an integrated fan-beam CT scanner. Leo says the system can provide six degrees of freedom for positioning and allows imaging to be performed at the treatment position. This approach is designed to maintain the patient's orientation between imaging and treatment and potentially support image-guided workflows.

The upright setup may also be useful for some patients who have difficulty remaining flat because of pain, breathing problems, limited mobility, or anxiety. The open configuration is intended to provide a less confined treatment environment while allowing the clinical team to remain engaged with the patient during positioning. These are potential patient-experience advantages rather than guarantees for every individual.

One of the notable features of the MEVION S250-FIT is its compact architecture. Mevion describes it as a proton therapy system designed to fit within an existing radiation therapy vault, with a self-shielded accelerator and integrated imaging capabilities. The manufacturer says the design can reduce some of the space and infrastructure requirements traditionally associated with proton therapy installations. Leo Cancer Care's Marie system is similarly designed around a fixed particle beam rather than a large rotating gantry. The patient is slowly rotated to change the treatment angle, while the beam remains fixed. According to Leo, this architecture can make particle therapy systems considerably more compact and potentially allow installation within existing treatment infrastructure. For FMRI, this could be particularly relevant because the planned system is intended to be installed within an existing building. The manufacturers and Fortis say the compact configuration can overcome some of the space and construction challenges associated with conventional gantry-based proton therapy facilities.

The system will also incorporate RayStation, the treatment-planning platform developed by RaySearch Laboratories. The planned configuration brings together proton treatment delivery, upright patient positioning, CT imaging, and treatment planning within the same clinical workflow. Mevion's S250-FIT platform supports pencil-beam scanning and image-guided treatment capabilities, while its large-bore CT is designed for imaging at the treatment position. The company has also described the platform as capable of supporting emerging technologies such as adaptive planning and proton arc therapy, although some of these capabilities remain under development or are not yet available for routine clinical use.

The planned installation would add another advanced radiation-oncology capability to FMRI's existing cancer-care infrastructure. Fortis identifies FMRI as a quaternary-care hospital with medical, surgical, and radiation oncology services and a multidisciplinary oncology program. Its radiation-oncology offering already includes advanced treatment modalities and proton therapy as part of its listed cancer-care services. The announcement also reflects a wider effort to make highly specialized cancer technologies more accessible within established healthcare centers. Proton therapy facilities have traditionally involved substantial infrastructure, shielding, space, and capital requirements. The compact, fixed-beam approach being introduced at FMRI is intended to address some of those barriers. The installation is not yet equivalent to the start of routine patient treatment. Regulatory approvals, site preparation, and implementation must be completed before the system becomes operational. Leo also notes that its Marie technology is still in the process of expanding into different markets and that clinical evidence for its specific upright technology continues to develop. If successfully commissioned, the FMRI installation could mark an important development in India's adoption of compact proton therapy and upright radiation-treatment technology, particularly for selected cancers where precise radiation delivery and protection of surrounding tissues are clinically important.
