The Union Ministry of Health and Family Welfare has conducted a review regarding the progress made in achieving the goal of malaria elimination by the year 2030, highlighting the requirement of intensifying the interventions in the affected districts. The meeting, which was presided over by Smt Aradhana Patnaik, who is the Additional Secretary and Mission Director of the National Health Mission (NHM), was attended by officials of the Ministry and the National Centre for Vector-Borne Disease Control (NCVBDC), along with state NHM missions and the district administration.

Malaria is a mosquito-borne infectious disease caused by Plasmodium parasites and transmitted primarily through the bite of infected female Anopheles mosquitoes. The infection commonly causes fever, chills, headache, sweating and weakness, while severe malaria can lead to complications affecting the brain, kidneys and other organs and may become life-threatening if treatment is delayed. Early diagnosis through appropriate testing and prompt treatment with recommended anti-malarial medicines are important to prevent severe disease and further transmission. Prevention focuses on reducing mosquito exposure and breeding, including vector-control measures, environmental management and community participation.

India has made considerable achievements regarding malaria reduction over the decade. As per the records shared by the Ministry, the country displayed a dip in malaria cases and deaths by nearly 80% between the years of 2015 and 2025. Furthermore, high-burden districts in India have also reduced from 152 in the year 2015 to 33 such districts. About 160 districts recorded no indigenous malaria cases from 2022 to 2025.

Even though transmission rates have reduced in general, a significant number of cases are still found in certain locations. The number of cases examined by the review is 33 districts in nine states and Union Territories, which represent 64% of all malaria cases reported as well as 54% of malaria deaths that occurred in 2025. Reviews at state and district levels were conducted in Mizoram, Odisha, Tripura, Assam, Andhra Pradesh, Andaman & Nicobar Islands, and Chhattisgarh, Jharkhand, and Maharashtra.

Smt Aradhana Patnaik emphasised that India should keep the high level of surveillance and apply area-specific tactics. States were advised to implement District Action Plans in all 33 high-burden districts and Village Action Plans in high-burden villages.

The meeting highlighted the importance of the “Test, Treat and Track” approach in reducing malaria-related illness and deaths. Delays in diagnosing malaria and initiating treatment were identified as important factors associated with avoidable deaths.

States were advised to strengthen active surveillance and ensure that diagnostic facilities and anti-malarial medicines remain continuously available, particularly in remote, hard-to-reach and vulnerable communities. Early identification of fever and timely treatment are expected to remain central to preventing severe disease and reducing further transmission.

The review was done on the quality of the surveillance data that is utilised in malaria surveillance. States were advised to look for unusually high Annual Blood Examination Rates (ABER) to detect the possibility of duplicate testing, repeated entries, and the inclusion of asymptomatic screening.

The Ministry also explained that the asymptomatic cases found through mass screening should be reported separately and should be excluded from the Annual Parasite Incidence (API) and ABER calculations. Accurate reporting is necessary for understanding the real disease burden and evaluating the malaria elimination programmes.

The ministry pointed out that the elimination of malaria hinges not merely on health services because environmental conditions are critical in enhancing mosquito reproduction and movement. Problems like waterlogging, the existence of mosquito breeding sites, and inadequate source reduction require collaboration beyond the health department.

States were thus encouraged to collaborate with rural developments, urban developments, and panchayati raj institutions to enhance environmental management and vector control actions.

Community participation is cited as an important aspect of the elimination campaign. The self-help groups, community volunteers, and panchayati raj representatives assist by early reporting of fever cases, determining waterlogged regions, pointing out mosquito breeding sites, and undertaking source reduction activities. While primarily noting that malaria cases and deaths have reduced drastically since 2015, the government is now working on high-burden zones.
