The U.S. FDA has published a white paper titled “Key Considerations for the Development and Use of Digitally Derived Measures for Clinical Investigations,” outlining how data collected through digital health technologies can be developed and evaluated for use as outcomes in clinical research. The accompanying FDA Voices article was published by Dr. Rick Abramson, Director of the FDA’s Digital Health Center of Excellence.

The U.S. Food and Drug Administration (FDA) is the federal agency responsible for protecting and promoting public health in the United States by regulating medicines, medical devices, biological products, food, vaccines and other health-related products. In healthcare research, the FDA also provides regulatory guidance for the development and evaluation of new technologies, treatments and clinical trial methods. Its recommendations help researchers, pharmaceutical companies, medical device manufacturers and digital health developers understand the evidence and safety requirements needed before products or technologies can be used in regulated healthcare settings.

Digitally derived measures (DDMs) are measurements generated from data collected through digital health technologies such as wearable devices, sensors, and other connected technologies. Depending on their intended use, these measures can potentially be used as clinical outcome assessments, biomarkers or components of composite endpoints in clinical investigations. They may capture aspects such as mobility, sleep, cardiovascular function and cognition over a patient's normal daily activities rather than only during periodic visits to a clinical site.

The FDA's document places particular emphasis on validation, as simply collecting large amounts of digital data does not make a measurement suitable for clinical research. Developers are expected to establish that the technology and resulting measure are appropriate for the specific clinical question and intended use.

The framework highlights three important stages. Verification examines whether the technology can make the intended measurement accurately and precisely. Analytical validation assesses whether the measure appropriately captures the clinical event or characteristic it is intended to assess. Clinical validation determines whether the digitally derived measure reflects a meaningful change in a patient's health or clinical status.

The FDA also recommends starting with the health aspect researchers want to understand, rather than beginning with a particular sensor, device or algorithm. Developers should define the concept of interest, target population and intended context of use before determining how digital technology should be applied.

Digital measures could also support decentralised clinical trials by allowing certain health information to be collected remotely and continuously. Compared with measurements taken during short clinical visits, appropriately designed digital measures may provide information about how a patient functions over longer periods and whether a change persists in everyday life.

The FDA's document is a U.S. regulatory framework and should not be interpreted as an Indian regulatory requirement. Its relevance for Indian pharmaceutical companies, CROs, researchers and digital-health developers lies in understanding how digital biomarkers and wearable-derived data may need to be evaluated when developing products or conducting clinical research involving the U.S. market.

The FDA has also stressed that patient input should be part of the development process, particularly when determining whether a digital measure captures something meaningful to patients
