The Central Drugs Standard Control Organization (CDSCO) has issued a circular advising the public, healthcare professionals, pharmacists, hospitals, and drug distributors to promote the safe and rational use of non-steroidal anti-inflammatory drugs (NSAIDs) and antibiotics. The advisory, dated 21 September 2026, highlights the potential risks associated with inappropriate or prolonged use of these medicines, including renal complications and antimicrobial resistance. Non-steroidal anti-inflammatory drugs (NSAIDs) are medicines commonly used to relieve pain, reduce inflammation and control fever. Antibiotics are medicines used to treat bacterial infections and should be taken only when clinically indicated and under appropriate medical advice.

Under the Drugs and Cosmetics Act, 1940, and the Drugs Rules, 1945, the sale and distribution of prescription medicines are subject to specified requirements. CDSCO notes that many NSAIDs are included under Schedule H, while most antibiotics are covered under Schedule H1. These medicines are therefore required to be used under appropriate medical supervision and according to the applicable prescription and labelling requirements.

The circular specifically advises the public not to take painkillers or antibiotics repeatedly, indiscriminately, or for prolonged periods without medical advice. Antibiotics should not be self-medicated or used for conditions where they are not clinically indicated, including most uncomplicated viral infections. Patients are also advised not to share prescribed antibiotics or use leftover medicines.

CDSCO has asked healthcare professionals to prescribe NSAIDs and antibiotics only when clinically indicated, after considering factors such as the patient's age, medical conditions, other medicines, and risk factors. For NSAIDs, the advisory recommends considering the lowest effective dose for the shortest appropriate duration, particularly for patients who may have an increased risk of renal impairment.

The circular also calls for responsible antibiotic prescribing in line with antimicrobial stewardship principles. Healthcare professionals are advised to consider the likely cause, site and severity of infection and, where appropriate, microbiological investigations and local susceptibility patterns. Unnecessary combinations, inappropriate antibiotic selection, incorrect dosing and unnecessarily prolonged treatment should be avoided.

Pharmacists, retailers and distributors have been advised to strictly follow the statutory requirements governing Schedule H and H1 medicines. They should not facilitate self-medication with antibiotics or prescription-only NSAIDs and must comply with the prescribed warnings and conditions of sale.

Hospitals and healthcare institutions have also been asked to strengthen systems for rational prescribing, prescription review, patient counselling and antimicrobial stewardship. State and Union Territory drug control authorities have been requested to create awareness among healthcare professionals, pharmacists, retailers and the public while ensuring compliance with applicable drug regulations. The CDSCO circular ultimately calls for coordinated action to encourage the safe, responsible and clinically appropriate use of NSAIDs and antibiotics, to reduce avoidable adverse drug reactions, renal complications and antimicrobial resistance.
