The American Academy of Neurology (AAN) and the American Headache Society (AHS) have issued an updated joint practice guideline on medications used to prevent migraine in adults. Published on August 31, 2026, the guideline updates recommendations that were last issued in 2012 and incorporates evidence from newer research and preventive treatments developed in recent years. The guideline has also been endorsed by the American Academy of Family Physicians.

Migraine is considered a long-term neurological disorder that leads to recurring attacks of headaches accompanied by nausea, sensitivity to light and noise, dizziness, changes in vision, and difficulty thinking. Attacks can happen from hours to days and negatively impact jobs, social life, and everyday activities. Acute medications are used during migraine attacks, and medications are prescribed regularly to prevent further attacks.

Preventive treatment is recommended for individuals who suffer four or more migraine attacks or four or more moderate-to-severe migraine attacks every month and those who experience significant impact of migraine on their job and daily activities. The recommendations include both episodic and chronic migraine classification; the latter is defined in the guidelines as a headache occurring 15 days a month at least, and at least 8 of them having migraine characteristics.

The revised recommendations note the existence of a larger number of preventive medications available now. This group of medications includes both traditional medications and recently developed classes of migraine-specific treatments. According to the recommendations, the fact that one certain treatment has not been effective for one patient is not sufficient for the conclusion about a possible ineffectiveness of an alternative preventive medication for another patient, since each medication works differently.

The recommendations state that when developing a treatment plan, one should take into account the effect of migraines on quality of life, the reliability of evidence available, potential side effects of the treatment, costs, and the type of medication to be used. The approach to preventive treatment that may be applied is taking an oral medication daily or on an every-other-day basis, as well as taking injections with the medication at certain intervals, for instance, every month or every three months. Several traditional medications, such as some antidepressants and medications reducing blood pressure, may have a positive impact on patients suffering from other diseases.

The organisations have stressed the importance of shared decision-making between clinicians and patients when selecting preventive therapy. After a new preventive medicine is started, clinicians should assess its effectiveness according to the timeframes recommended in the guideline and consider whether the treatment is providing meaningful improvement.

The updated guideline reflects the growing number of treatment choices available for people living with migraine and aims to help clinicians select preventive therapies based on individual patient needs and the available scientific evidence. The recommendations are intended to support neurologists, primary-care physicians and other healthcare professionals in providing evidence-based care for adults with frequent or disabling migraine.
