An 8-year-old child in Ahmedabad has successfully recovered from fulminant H1N1 myocarditis, a severe and rapidly progressive inflammation of the heart muscle associated with H1N1 influenza infection, following advanced cardiac support at Apollo Hospitals, Ahmedabad. The child was treated by a multidisciplinary team led by Dr. Jay Shah, Pediatric Intensivist; Dr. Deepti Kakkar, Pediatric Cardiac Surgeon; and Dr. Pavan Patel, Pediatric Cardiologist, along with the PICU, cardiac anesthesia, perfusion, nursing and other specialist teams. The condition caused profound impairment of the heart's pumping function, with the child's left ventricular ejection fraction (LVEF) falling below 10%, along with refractory cardiogenic shock. Cardiogenic shock occurs when the heart becomes too weak to pump sufficient blood to meet the body's needs. In this case, the child's condition remained critical despite maximum medical treatment, requiring advanced mechanical circulatory support.

The child was therefore urgently placed on central veno-arterial extracorporeal membrane oxygenation (VA-ECMO). VA-ECMO is an advanced form of temporary life support used in patients with severe cardiac and circulatory failure. The system takes over part of the heart's workload by supporting blood circulation and oxygen delivery to the body, allowing the severely weakened heart time to recover. In cases of fulminant myocarditis, this temporary support can be particularly important when cardiac function deteriorates rapidly, and conventional medical treatment is insufficient.

The child remained on central VA-ECMO for approximately 84 hours. During this period, the heart showed progressive recovery, allowing the medical team to successfully remove the ECMO support, a process known as decannulation. Following decannulation, cardiac function continued to improve. The child's LVEF increased from less than 10% to 51%, representing substantial recovery of left ventricular function. The child was subsequently discharged in a stable condition and is continuing rehabilitation.

The successful management required coordinated care from a multidisciplinary pediatric team. Pediatric critical care specialists were involved in intensive monitoring and management, while the pediatric cardiac surgery team managed the surgical aspects of central VA-ECMO. Pediatric cardiology supported the assessment and management of cardiac function and recovery. The child's care also involved cardiac anesthesia, perfusionists and PICU nursing staff, along with specialists from neurology, nephrology and hematology. Physiotherapy, respiratory therapy and nutrition teams contributed to the child's recovery and rehabilitation.

Fulminant myocarditis is an aggressive form of myocarditis in which inflammation of the heart muscle can develop rapidly and severely impair myocardial contraction. When the heart's pumping ability falls substantially, patients can develop low blood pressure, inadequate organ perfusion, and cardiogenic shock. The condition therefore requires rapid assessment and intensive supportive management. In selected patients with severe cardiac failure who do not respond adequately to medical therapy, VA-ECMO can provide temporary mechanical circulatory support while the myocardium has an opportunity to recover.

The improvement in this child's LVEF from below 10% to 51% demonstrates the potential for significant myocardial recovery following severe acute cardiac dysfunction when timely advanced support is provided. The case also underscores the importance of early recognition of fulminant myocarditis, rapid escalation to mechanical circulatory support when clinically indicated, continuous intensive monitoring, and multidisciplinary coordination in the management of critically ill children.
